The capture stage decides the quality of everything after it. AssemblyAI's guide warns that "if the transcript logs the patient's reported symptom as the clinician's observation, or turns 'hydralazine' into 'hydroxyzine,' the LLM downstream will confidently structure wrong information into a clean-looking note." Freed describes the same pipeline from the clinician's side on its ambient clinical documentation page: it "captures the entire conversation and filters what matters," and "by the time the visit ends, the draft is waiting."
What ambient AI capture records, and why the patient's voice matters
Ambient AI capture records the room, not just the clinician, and that is the main difference from dictation software such as VoicePrivate Healthcare Edition. Where does the audio physically go? AssemblyAI's advice: "Vendors who answer crisply have thought about it. Vendors who answer with a badge have not."
Recording a conversation also raises consent. Software companies that build their own ambient capture pay per hour of audio instead of per clinician. AssemblyAI lists $0.21 an hour for recorded audio, or $0.36 an hour with its medical setting, in its ambient scribe guide checked 3 October 2026, and adds the honest caveat that "transcription is the cheap part. Is ambient AI capture worth it? What the evidence says
Ambient AI capture is aimed at a real problem. In the time and motion study by Sinsky and colleagues in the Annals of Internal Medicine, "for every hour physicians provide direct clinical face time to patients, nearly 2 additional hours is spent on EHR and desk work within the clinic day" (PubMed abstract, 57 physicians in 4 specialties, 2016). Whether ambient capture pays for itself is less settled:
